Summit Independence Options for People with Brain Injuries |
|
What will my life look like now that I have had a brain injury? Will I be able to live on my own? Who can support me in my community?
Summit is designed to answer these questions in a way that works for you. This program serves brain injury survivors in the Waterloo Wellington area. Summit offers a variety of housing options such as transitional apartments, with supports designed to meet your needs and goals.
You can start at the level of support that is right for you, climb at your own pace, and “Summit” to higher levels of independence. Helping you in your climb is your Summit team which may include Rehabilitation Support Workers, Attendant Care Workers, Regulated Health Practitioners, and your family and friends.
Facilitated Intake Process: We have introduced a new facilitated intake process to assist survivors with the application process. Please contact Bonnie Wilson at 519-742-9424, Ext. 207, or bonnie@phwaterloo.org |
