Brain Injury (ABI) Services

 

Facilitated Intake Process

 

Participation House offers a single point of access for Brain Injury clients. Bonnie Wilson, Intake Coordinator, will assist with the following:

· Information regarding the various programs available to brain injury survivors

· Application and Referral process

PHWW offers a continuum of services to brain injury survivors in partnership with the Brain Injury Association of Waterloo-Wellington and Key Rehab Services Inc.

· The Opportunity Centre - a drop-in style day program in downtown KW for survivors

· Beginnings  -  a facilitated day program for brain injured individuals with physical needs

· -a drop-in style day program in downtown KW for survivors

· Outreach Services  -  support in clients' homes or in the community

· -a drop-in style day program in downtown KW for survivors

· ABI Group Home  -  24-hour support  with a focus on independence

· -a drop-in style day program in downtown KW for survivors

· Summit  -  supported independence options for ABI survivors

· -a drop-in style day program in downtown KW for survivors
